USA Visa Application Requirements

For Job Holders:

• Valid passport (minimum 7 months) and old passport (if available)
• Visiting card
• No Objection Certificate (NOC)
• BMDC certificate for doctors (scanned copy)
• BAR Council certificate (one photocopy)
• Marriage certificate (if spouse's name is not mentioned in the passport)
• Government Order (GO) and Note Verbale or Letter of Introduction (LOI) issued by the Ministry of Foreign Affairs, Bangladesh
• TIN certificate and tax return copy
• Scanned copy of passport bio page
• List of previous countries visited
• Educational institutions attended (names and details)
• Parents and spouse information (name, date of birth, birth place)
• Current employer or school details (names, addresses, and phone numbers)
• Monthly salary in local currency and designation (if employed)
• Previous employment details (employer name, address, phone number, job title, supervisor's name, dates of employment)
• NID or birth certificate (one photocopy required)
• Salary certificate
• Property documents
• Blue book photocopy of vehicle (if applicable)
• Documents for any other income
• Recent color photograph (2x2 inches, matte paper)
• Financial solvency documents (FDR, shares, bonds, savings, security deposits, cash investments)

For Businesspersons:

• Valid passport (minimum 7 months) and old passport (if available)
• Personal or company bank statement from the last 6 months (minimum balance of BDT 70,000 for each applicant)
• Renewal of trade license (notarized and translated to English)
• Visiting card
• Marriage certificate (if spouse's name is not mentioned in the passport)
• Memorandum for limited company (Form Page XII, one photocopy)
• TIN certificate and tax return copy
• Personal bank solvency certificate
• Scanned copy of passport bio page
• List of previous countries visited
• Educational institutions attended (names and details)
• Current employer or school details (names, addresses, and phone numbers)
• Monthly salary in local currency and designation (if employed)
